The following twelve individual club members qulaified for the Eastern Championships
Calling all clubs – please join in on this day of hands-on, team fun!!
Location: Hannaford Career Center (adjacent to Middlebury High School)
51 Charles Ave.
Middlebury, VT
Time: Registration: 8:30-9:00 am.
Competitor and volunteer briefing: 9:00-9:15
Competition:9:20 am.-3:00 pm.
Refreshments and entertainment 3:00- 3:30 pm.
Awards 3:45 pm.

Required paperwork (forms can be found on USPC website under “forms” tab)
– Entry Form (appendix A from quiz rulebook), 1 per team
– Medical Release form (original signature), 1 per competitor
– Competitor activity /Rally release, 1 per competitor (this is a 2 page document and requires signatures on both pages)
– Chaperone duties form, 1 per team
– Volunteers (at least one per team, can not be the chaperone) (no form for this, put this information on bottom of entry form)
– Club check for entry fees ( $25 per competitor)
Packets must be postmarked by 2/16 or will receive a $25 late charge.
